Checklist for Freshmen Applicants

  • Original Form 138/SF9-SHS (Learner’s Progress Report Card)
  • Original Form 137/SF10-SHS (Learner’s Permanent Academic Record)
  • Certificate of Good Moral Character
  • PSA (formerly NSO) Issued Birth Certificate (Photocopy)
  • 1 piece of a passport size picture with white background

Checklist for Transferees Applicants

  • Original Form 138/SF9-SHS (Learner’s Progress Report Card)
  • Original Form 137/SF10-SHS (Learner’s Permanent Academic Record)
  • Certificate of Good Moral Character
  • PSA (formerly NSO) Issued Birth Certificate (Photocopy)
  • 1 piece of a passport size picture with white background

Checklist for Non-Resident Foreign Applicants

  • Letter of Application/Intent, address to the registrar
  • Six (6) copies of the Original Personal History Statement (PHS) and Student’s Admission
  • Review Checklist (REG – 2C) with 2x2 colored picture of the application
  • Original and Two (2) copies of all pages of the academic records (diploma, report cards, mark sheets, etc.) of the applicant with seal of the Philippine Embassy or Consulate in the country of origin of the applicant
  • Original and Two (2) photocopies of the affidavit of support
  • The Statement of Adequate Financial Capabilities
  • Three (3) photocopies of the passport (with photo, information and birth certificate of the applicant) authenticated by the Philippines Consul
  • Alien Certification Registration (ACR)
  • Photocopy of Student Visa

Checklist for Graduate School Applicants

  • Two sets of transcripts of records (one original & one photocopy).
  • Two written recommendations or endorsements (from current or former professor).
  • Certificate of work experience indicating the nature of job and length of service (from employer)
  • Two I.D. photos (1pc 2x2 and 1pc 1x1)
  • If married, photo copy of a marriage contract

guidance for enrollment

Guidance for Freshmen Applicants, Transferees, and Non-Resident Foreign Applicants:

  1. Submit the application requirements at the Admissions Office
  2. Secure Registration Form at the Admissions Office or download here
  3. Pay school fees at the Finance Office
  4. Proceed to advisement area for further instruction

Guidance for Graduate School Applicants:

  1. Fill-out an Application Form and Enrollment Form and secure the necessary signatures at the Graduate School office in CB Building
  2. Take the Written Examination
  3. Proceed to the Dean’s/Director's Office for an interview
  4. Submit the admissions requirements to the Graduate School Office
  5. Proceed to the Registrar's Office for approval of enrollment form
  6. Proceed to the Accounting Office for payment
  7. Proceed to the Library and Media Center (LMC) to secure your Library Card by presenting your Official Registration Form to the Head Librarian
  8. Proceed to the I.C.T. and present your Official Registration Form to secure your SCHOOL I.D.
